A record of the care a patient receives at Stephens Memorial Hospital is retained in the Health Information Management (HIM) department. This record of care, called your Protected Health Information (PHI), is kept in strict confidence and will not be released without the patient’s written consent, except as required by law.
No PHI will be released without picture ID with signature for confirmation of identity.
