Swing Bed /
Skilled Nursing Program

Lets you stay close to your home and family while receiving care.

What is the Swing Bed Program?

The term swing bed refers to the way Medicare reimburses for the care provided to the patient. The patient stays in the same bed, but the level of care swings from acute to skilled rehabilitation.
The purpose of a swing bed program is to provide additional inpatient care to those needing extra time to heal or strengthen before returning home.  This program assists you in reaching your highest potential for health and independence after hospital discharge.

What type of
health care will I receive?

Since patients’ needs vary, the type of healthcare varies as well. When the patient is first admitted, they will be assigned their own room and a plan of care will be developed. Even though the patient will not see the physician on a daily basis, they will still be monitored by nursing staff. The nursing staff will report any changes or additional needs to the physician. The plan of care will be reviewed every 5 to 7 days.

Skilled Nursing

Our nursing staff provides 24 hour, skilled care including assistance in daily activities, therapy, wound care, and patient/family teaching.

Physical Therapy

Physical therapists assist in recovery by helping you gain back your strength so you can resume life activities.

Occupational Therapy

The goal of occupational therapy is to restore your daily living and increase independence.

Speech Therapy

Our speech therapist works closely with patients in need of cognitive relearning and swallowing issues to help bring patients back to full recovery.

Your Physician

You will receive frequent visits with your physician who personally directs your care.

Respiratory Therapy

From oxygen therapy and breathing treatments to tracheotomy care, respiratory therapists assist patients to breathe easier and maintain optimum oxygenation.

IV Therapy

IV therapy provides rapid delivery or access to the bloodstream for rehydration, nutritional support, or medication administration.

Nutrition is vital to a quick recovery. Our staff wants to meet both your personal and dietary needs.

Who Qualifies?

Patients must have at least a 3-day stay in the last 30 days as an acute patient and must also need some form of daily skilled nursing or skilled rehabilitation service, which can only be provided in a skilled nursing facility.

How Do I Pay?

  • If you have Medicare Part A, your coverage for these services is the same as if you were in another Skilled Nursing setting.  If you meet the criteria and have not used any Skilled Nursing coverage in the last 60 days, the first 20 days of your care will be covered in its entirety.  Care after the 20th day and up to 100 days will be covered but will require you or your secondary insurance to pay the co-insurance.  To meet the criteria, you must:
    • Have stayed overnight as an inpatient in a hospital for three consecutive nights within the last 30 days.
    • Have a determination by a doctor or other healthcare provider that you need ongoing care for a condition that was also treated while you were in the hospital and that must be given by or under the supervision of skilled nursing or rehabilitation staff.
  • If you have a Medicare Advantage plan, the criteria for Swing Bed coverage will be the same as with Medicare Part A.  However, these plans may not require an inpatient stay, but will need to be pre-authorized by your insurance.  Your individual plan may have a copay during the first 20 days.
  • Non-Medicare insurance plans that cover Skilled Nursing care generally also follow Medicare guidelines and cover Swing Bed, but your coverage and benefits will be specific to your plan.
